7. Lost, damaged or stolen cards

If you need a replacement card please call us on 01202 261306.

To replace a lost, damaged or stolen card, you will need to reapply for a new card.

Please complete a new application form and take this, together with your proof of eligibility, a photo and payment to one of the enrolling centres, or alternatively apply online.

If you find an ATL&L card, please return it to:

Borough of Poole, Recreation Development, Upton House, Upton Country Park, Poole BH17 7BJ.
