The Poole Community Learning Disability Team (CLDT) is an integrated team of health and social care workers from the Borough of Poole and Dorset Health University Foundation Trust (DHUFT). The team consists of Social Workers, Learning Disability Community Nurses, Occupational Therapists and Physiotherapists. It also has close links with the Consultant Psychiatrist, Consultant Psychologists and Speech and Language Therapist. We also have links with the Community Dietician, district nurses, GPs and local hospitals as well as the voluntary sector, for example:

Learning disability can be described as an arrested or incomplete development of the mind. People are born with this condition or develop it very shortly after birth.

The CLDT only works with adults who have a diagnosed learning disability. Individuals who have developed a brain injury, either as a result of an accident or from substance misuse should contact Healthpoint for information on brain injury organisations.

You, family members, advocates and professional colleagues can refer you to the team. Once accepted, you will be assessed in a person centred way by a health or social care worker so as to identify your needs and goals and how they can be met. This could be in a variety of ways, for example:

  • through existing community groups
  • friends or family
  • adult education
  • commissioned services and direct payments
  • support from health care staff in the team
  • or a combination of the above
