Dorset Dementia Partnership 2016 - 2018

Purpose

Dementia will be the most significant long term condition facing older people in future years. It is important to develop and implement an improvement plan that builds on current progress and good practice and importantly takes a whole system approach across health and social care.

The aim will be to build further on the dementia strategies both local and national to increase the identification and diagnosis of dementia and improve the care, treatment and support offered to people with dementia and their families and carers. 

This is a collaborative plan developed with a range of stakeholders and is a working document which will continue to evolve as further innovations develop.
