You will need to complete and return your application form with the following:

The processing of your application may be delayed if you omit to send the following:
- A cheque or postal order for the administration fee of £2.00, made payable to 'Borough of Poole'.
- Two recent passport-size photographs of yourself. Please print your name on the back of the photos and sign the box at the bottom of the back page of the form, as your signature and photo will be included on your badge. (please do not attach with pins)
- I have enclosed the relevant documents to support my application under questions 1-4 (We need to see the length of your DLA award as this affects the expiry date of your badge)
- I have had a Blue Badge issued by another Authority (please indicate on form who issued it and the expiry date)
- I have signed the cut off box at the bottom of the blue application form (or someone has on my behalf in my name).
- Please complete the separate checklist and return it with the form.
Please note:
- There is no need to send any documents with the form if you have been registered blind by Borough of Poole. If you have been registered blind by another authority, please enclose your Blind Registration Card.
- If you have answered 'yes' to any or all of questions 2 to 4 on the form, please send with your form, photos, the fee and either a photocopy of or an original Social Security or Benefit Agency document showing that you currently receive either.
We will also need to see the end date of your award as this will determine the expiry date of your Blue Badge.
- Disability Living Allowance at the higher rate for getting around, (DLAHRMC).
- Vehicle Licence Tax Exemption (DLA404),
- Mobility Car Award Allowance (ie. Motability Scheme), or
- War Pensioners' Mobility Supplement. - If you do not have a copy you can get one from the Mobility Helpline on 08457 123 456.
- If you have been issued a badge by an Authority other than Borough of Poole, please include details with your application (eg. issuing Authority, expiry date, badge number) and, if possible, a photocopy of the badge front.
- If you have answered 'no' to question 2 - question 4 please complete Part C, Part D or Part E as this information will be forwarded to your GP for them to complete a form. Please supply details of your GP in Part F.
The leaflet 'The Blue Badge Scheme' will be sent to you with your badge. Please read it carefully.
