You can obtain a copy birth certificate for a birth that was registered in Poole by downloading and completing this application form, for example if you have lost your original birth certificate.
The cost of both full and short birth certificates are £10.00 each. We cannot issue a certificate until we have received payment. Cheques or postal orders should be made payable to Borough of Poole.
