Poole Intermediate Care Service (PICS)
Poole Intermediate Care Service (PICS) is a new joint service for adults and older people in the Borough of Poole designed to:
- to prevent unnecessary admission to hospital/residential care
- to facilitate rapid discharge from hospital
- support carers at times of crisis.
This service may include Senior Nursing and Social Work staff undertaking assessments, providing treatment and devising ongoing care packages; with front line practical support provided by a team of Intermediate Care Assistants. In practice an older person might receive support to dress, wash, cook, take medication, mobilise, in addition to nursing tasks appropriate to their condition. It is funded by the Borough of Poole and the Bournemouth and Poole PCT

Referral
Referral to the Service can be made via the Adult Social Services Helpdesk on 01202 633902, email sshelpdesk@poole.gov.uk, although it is more commonly as a consequence of the teams relationship with hospital and GP surgery staff. There is no dedicated referall form.
